About the role
An exciting opportunity has arisen for a Locum Consultant Geriatrician to join the Community Care of the Elderly and Frailty teams in Chepstow Community Hospital. The successful candidate will provide senior clinical leadership to the inpatient community medicine ward and the Monmouthshire Rapid Frailty Response service, supporting timely decision-making for the older and/or frail adult.
The post holder will play a central clinical leadership role in promoting multi-disciplinary working, the use of evidence-based practice and ensuring that the principles of clinical governance are fundamental to the design, delivery and evaluation of new and existing models of care.
This is a hybrid role that centres on early intervention and proactive management of older and/or frail individuals enabling care to be provided in the most appropriate setting at the right time.
To support the inpatient medical ward as named physician responsible for patient care
To perform a key role in discharge planning and co-ordinating approaches with relatives and other agencies
To liaise with patients, relatives/carers and members of the multi-disciplinary team to ensure patients receive a high-quality service and are kept informed of progress as appropriate
