Salaried General Practitioner

The post holder will work as a Salaried General Practitioner within The Limes Medical Centre, providing high-quality, safe and patient-centred medical care to the practice population.

The successful candidate will work collaboratively with the practice team and wider multidisciplinary team to provide effective management of acute and chronic conditions, preventative healthcare and appropriate onward referral.

Clinical Duties Undertake face-to-face and telephone consultations as required. Assess, diagnose and manage a wide range of acute and chronic medical conditions. Develop appropriate management and treatment plans in partnership with patients. Prescribe safely and appropriately in accordance with national and local guidelines. Review and act upon laboratory, imaging and other diagnostic results. Make appropriate referrals to secondary care, community services and other healthcare professionals. Provide appropriate follow-up and ongoing management of patients. Undertake home visits where clinically appropriate and in accordance with practice arrangements. Provide preventative healthcare, health promotion and lifestyle advice. Support the management and review of patients with long-term conditions. Recognise and appropriately manage patients presenting with urgent or emergency conditions. Maintain accurate, contemporaneous and comprehensive clinical records using EMIS Web. Ensure appropriate clinical coding and documentation is completed.
