Advanced Clinical Practitioner - Ashton GP Service Advanced Clinical Practitioner £57,625 to £71,281 per annum, pro rata, depending on experience, yearly salary increments, NHS Pension provider. An exciting opportunity to work in a supportive, community focused and a well establishedpracticefor an Advanced Clinical Practitioner at Ashton GP Service. You will be supported by strong clinical leadership and a collaborative multidisciplinary team. The hours for this role are 30 hours per week, to be worked on Wednesday and Friday between 08:00 and 18:30. The Role We are seeking a confident, autonomous Advanced Clinical Practitioner to work at Ashton GP Service. You will play a key role in delivering safe, effective and patient centred care within a supportive multidisciplinary team. Your responsibilities will include:

Managing acute and routine presentations across both sites Undertaking clinical assessments and diagnostics Prescribing independently Supporting continuity of care for patients with long term conditions Working collaboratively with GPs, nurses and the wider MDT Contributing to service development and quality improvement

Person specification

Essential

Advanced clinical practice skills and competence. Management and treatment of patients with acute exacerbation of long-term conditions. Management of patients with complex needs. Clinical examination skills and competence. Clinical leadership skills and competence. Communication skills, both written and verbal. Negotiation and conflict management skills. Change management experience and evidence. Highly motivated. Flexibility. Enthusiasm. Team Player.
Experience of working as senior clinician. Accountability of own role and other roles in a practitioner-led service. Experience of leading/managing quality improvement.
Registered first level nurse, paramedic, pharmacist or other AHP with MSc Advanced Clinical Practice or credentialing qualification or equivalent. V300 Non-Medical Prescriber (gateway dependent) dependent on professional eligibility.

Desirable

Teaching and mentorship in a clinical setting. Resource Management.
Experience in leading/managing a team of nurses. Experience of working in an unscheduled care service. Experience of having worked in Primary Care.
Mentor/Teaching qualification. Clinical supervision training and experience.
