About the role
We are seeking an experienced and highly motivated Advanced Nurse Practitioner (ANP) or Advanced Clinical Practitioner (ACP) to join our primary care team for 3 days per week. This is an exciting opportunity for a clinician with strong autonomous practice skills to work within a supportive, multidisciplinary environment. The successful candidate will provide high-quality, patient-centred care, including assessment, diagnosis, treatment, and management of both acute and chronic conditions.
The ANP will work independently and collaboratively to deliver a high standard of clinical care. Key responsibilities include:
Assess, diagnose, plan, implement, and evaluate treatment/interventions for patients presenting with undifferentiated and undiagnosed conditions Manage acute presentations, minor illness, and long-term conditions Prescribe medications in accordance with local and national guidelines (if qualified as an independent prescriber) Order and interpret investigations such as blood tests, imaging, and ECGs Work autonomously within scope of practice and professional guidelines Provide health promotion, education, and preventative care Contribute to practice development, clinical governance, and quality improvement initiatives Support and mentor junior staff where appropriate Maintain accurate and contemporaneous clinical records
