Main duties of the job Responsible for managing a caseload and addressing a wide range of healthcare needs within a primary care setting. Work collaboratively with clinical and non-clinical colleagues to ensure the highest standards of care for all registered patients. To actively contribute to practice planning and decision-making processes. This role therefore encompasses both clinical and organisational responsibilities. Screens patients for disease risk factors and early signs of illness. Be available for 'on-call' duties to answer patients' telephone queries or carry out emergency home visits, or surgery consultations as appropriate. This also includes reading and actioning the hospital and other correspondence that comes in on the day Carry out procedures in a safe manner by following best practice and infection control policies. Work effectively with individuals in other agencies to meet patients' needs.
