As an GP/ Specialty Doctor, you will join the Wiltshire Hospital at Home (Virtual Ward) service in a senior clinical role, delivering medical assessment and decision-making for patients receiving acute care at home. The role focuses on admission avoidance, early discharge, and person-centered frailty care. Responsibilities include clinical assessment, management of patients, supporting the multidisciplinary team in delivering Comprehensive Geriatric Assessment (CGA), and promoting safe discharge. Candidates must have a background in Geriatric Medicine, Acute Medicine, General Medicine, or General Practice and experience with patients living with frailty and complex comorbidity. The role is community-based, requiring regular patient visits and flexibility across BSW for service continuity.
