Job Overview This role offers a local GP the opportunity to join a supportive Primary Care Network, helping them build confidence and develop skills beyond traditional general practice. The GP will work under the Additional Roles Reimbursement Scheme (ARRS), supporting the delivery of PCN DES Enhanced Health in Care Homes. The role will focus on providing proactive person-centred medical care for our care home residents, housebound or frail patients within the SWAN PCN footprint including our aligned care homes and working closely with our multidisiplinary team. The role will be based in SWAN PCN practices and the care homes locally that are aligned to the PCN.
Main Responsibilities Clinical Duties Provide face-to-face and telephone consultations for routine and urgent care for patients in local care homes and for patients with frailty. Conduct patient care home visits and ward rounds Manage patient care, including investigating and reviewing results, developing care plans, and making referrals to community, voluntary and secondary care or other providers. Issue and sign repeat prescriptions, ensuring they align with national/local guidelines. Administer and handle patient-related queries, test results, referral letters, and other tasks in a timely manner. Record clear, contemporaneous consultation notes into S1 clinical system. Ensure safe, evidence-based, and cost-effective care, in line with Practice/PCN disease management protocols and NICE guidance. Independently prescribe medications within your scope of practice. Review and manage patient medications following appropriate policies and local clinical guidelines. Utilise other available clinical and support staff in delivering a holistic and patient centred care. Actively participate in MDT and care conference meetings, as required. Contribute to the delivery of multi-disciplinary proactive care for complex patients at greatest risk of deterioration and hospital admission.
