About the role

This is a replacement 10 PA for the In-patient Rehabilitation Ward, Willow Ward in West Park Hospital. The post provides excellent opportunity for experience in both community and inpatient rehab work. Willow Ward is an inpatient rehab ward located in West Park Hospital in Darlington, South Durham. Willow Ward is a high dependency high support rehabilitation & recovery ward with 15 beds. The patients are working age adults who are highly symptomatic, with multiple or severe co-morbid conditions, significant risk histories and challenging behaviours. Most will be detained under the Mental Health Act. The resident doctor is expected to also cater to the needs of the patients on this unit. It must be emphasized that most admissions to rehab units are planned, so accordingly there is less need for emergency admissions however we have the capacity to do so depending on the nature of the referral and robust screening and assessment processes. The approximate length of stay for Willow ward is approximately 12-18months.

Clinical duties of post holder To complete new patient assessment for patients admitted to Willow ward, including taking a thorough history, mental state examination and complete physical examination in line with the trust policy. This also included making an initial assessment of risks and forming an initial management plan in collaboration with the admitting nurse. To ensure continuous monitoring of physical health of patients admitted to willow ward and where necessary to arrange further investigation or referral as required. To attend the 24 hour Morning Report Out meeting when key decisions are made about patient care and the management plan, risk and leave arrangements are reviewed on Willow Ward. To liaise with community, inpatient, and specialist services within the Trust as is clinically appropriate. To adhere to good and timely record keeping, including making specific comment about mental state examinations, risk assessment, management and the plans agreed. To complete timely transfer of care documents on willow ward. To respect the rights and the dignity of patients and to consider the individual and cultural needs of patients and work with families and carers in the best interest of the patient. To conduct medication reviews for psychotropic medications and to adhere to evidence-based prescribing and monitoring of pharmacological treatment. To adhere to guidelines of monitoring of physical health of patients on psychotropic medications.

Person specification

Essential

Able to demonstrate adequate skills in written/spoken English that enable effective communication with colleagues and service users .
Knowledge and understanding of the NHS Healthcare Leadership Model (HLM).
Able to show essential behaviours on the 9 dimensions of the HLM.
MB BS or equivalent medical qualification.
Fully registered with the GMC with a licence to practise at the time of appointment.
At least 4 years postgraduate experience, including at least 2 years in a core psychiatry training programme in UK or abroad
Approved under S12 of MHA OR able to achieve within 6 months of appointment.
Able to show commitment to Trust values of respect, involvement and well-being.
Able to work co-operatively in a multi-disciplinary team and to delegate appropriately.
Evidence of annual appraisal and commitment to appraisal and revalidation.
Able to demonstrate good clinical skills and knowledge in psychiatry with bio-psycho-social perspective.
