Coordinate and lead the delivery of a wide range of nursing services, including chronic disease management (e.g. diabetes, asthma, COPD), immunisations, wound care, cervical screening, and health promotion. Work collaboratively with GPs, other clinical staff, and external health and social care partners to ensure integrated, high-quality care. Ensure compliance with current clinical guidelines, infection control policies, and national/local protocols. Contribute to audits & risk assessments. Support the development and implementation of practice policies, protocols, and procedures in line with best practice and regulatory requirements.
