Salaried GP FSDEC

Frailty SDEC (f-SDEC) is a service focussed on supporting frail older adults and aims to treat/manage their conditions within the service. The team is made up of Advanced Practioners, Senior Nurses and Doctors. The postholder will work as part of a multi-disciplinary team reviewing patients and will work alongside the Acute Frailty team, Acute Medical Unit, Acute Hospital at Home Service and the Emergency Department. The post holder will be supported by Senior Medical (Consultants)
