South Fulham PCN has an exciting opportunity for a GP to work within our thriving PCN.

Please note that as an organisation, we are unable to offer visa sponsorship.

We are seeking a GP to support our PCN and practices. The successful applicant will be expected to work across more than one practice and should be capable of independent travel and the ability to work flexibly to meet PCN and practice needs. The post-holder will manage a caseload and deal with a wide range of health needs in a primary care setting, ensuring the highest standards of care for all patients. We are keen to attract the right candidate, and will work with you to agree what works best for the practices, PCN, and appointed person we are therefore approaching recruitment with an open mind on the specifics of how the role will function, be it rotating across practices, running a special interest clinic or working alongside our e-hub team.

South Fulham Primary Care Network (PCN) is a forward-thinking network delivering care to a patient population of around 64,000 residents across our seven practices. We aim to provide exemplary patient care, finding innovative integrated solutions to continuously improve what we offer. Our PCN and practice teams have positive and supportive working relationships, and we have a strong multi-disciplinary ARRS team which includes Clinical Pharmacists, Social Prescribers, Paramedics, Health and Wellbeing Coaches, Care Coordinators, an FCP and a dietitian. We work with our practices, local communities and with the wider health and care system to deliver high quality clinical services to patients. Our vision is that together we will deliver the best health and wellbeing outcomes for our residents and to work collaboratively across primary care and with wider partners as part of the integrated Hammersmith & Fulham borough-based partnership. Essential

A self-starter
Adaptable to different work environments
Experience of working within the NHS, with general public & team working
Good time management and highly organised
Ability to use initiative, judgement, resourcefulness, common sense and local knowledge
Interest in quality improvement work
Able to obtain and analyse complex technical information
Recognise priorities when problem solving and identify deviations from the normal pattern and is able to refer to seniors when appropriate
Able to work under pressure and to meet deadlines
Produce timely and informative reports
Work effectively independently and as a team member
Demonstrates accountability for delivering professional expertise and direct service provision
Eligible to work in the UK
GMC registered
Other postgraduate qualifications (Minor Surgery, Education, Family planning etc)
