About the role
We're looking for a motivated and experienced GP to join our friendly, multidisciplinary primary care team across the Hammersmith & Fulham Health Centres, based at Charing Cross Hospital and Hammersmith Hospital. This is an exciting opportunity to become an integral part of a team committed to delivering high-quality, patient-centred care and continuously improving the services we provide to our local community. As a Salaried GP, youll have the opportunity to use and develop your clinical skills while working closely with a wide range of healthcare professionals. Youll provide the full range of General Medical Services and play an important role in shaping and maintaining excellent standards of care across our Centres. Were seeking a committed, enthusiastic and experienced GP with a broad range of clinical experience and skills. The role is offered for a minimum of 4 sessions per week, providing an excellent opportunity for a GP looking for a rewarding role within a well-established primary care environment. Youll also need to be a trained Mental Health First Aid Champion or Freedom to Speak Up Champion, reflecting our commitment to creating a workplace where colleagues feel supported, valued and able to thrive.
Provide a full range of medical services as defined in the GMS contract, additional and enhanced services where appropriate. Make professional, autonomous decisions in relation to presenting problems whether self-referred or referred from other healthcare professionals within the organisation. Assess the healthcare needs of patients with undifferentiated and undiagnosed problems. Screen patients for disease risk factors and early signs of illness. Develop care plans for health in consultation with patients and in line with current practice; disease management protocols, provide counselling and health education. Maximise the effective use of current clinical computer systems. Record clear and contemporaneous consultation notes to an agreed standard. Compile and issue computer generated acute and repeat prescriptions (avoiding handwritten prescriptions whenever possible). To take part in areas including clinical auditing and data collection. Contribute to QOF and Northwest London Standard requirements and recording & code data appropriately Attend team meetings and other clinical meetings as necessary.
