The post-holder will manage a caseload and deal with a wide range of health needs in a primary care setting, ensuring the highest standards of care for all registered and temporary patients. The post holder will have ongoing responsibility for a specific group of patients registered under their care. This arrangement includes the following key points: Continuity of Care: The post holder will be the primary point of contact for these patients, allowing them to build a long-term relationship, better understand patients medical histories, and provide more personalised and consistent care. Clinical Responsibility: The post holder is clinically responsible for managing the healthcare needs of patients on their list. This includes preventative care, diagnosis and treatment of illnesses, chronic disease management, and referring patients to specialist services when needed. Patient Trust and Familiarity: Patients benefit from seeing the same doctor over time, which can improve communication, trust, and health outcomes. The doctor becomes familiar with each patients medical, psychological, and social background. Administrative and Record-Keeping Duties: The post holder will maintain accurate and up-to-date records for their patients, ensuring that all information related to diagnoses, treatments, and follow-up is well documented.
