Making professionally autonomous decisions in relation to presenting problems Consulting with patients in the surgery, during home visits, and via telephone or online consultations Undertaking triage calls, triage visits, and checking/signing repeat prescriptions Screening patients for disease risk factors and early signs of illness Developing care plans in consultation with patients and in line with current practice disease management protocols Providing health education where appropriate Admitting or discharging patients to and from the caseload and referring internally or externally as appropriate Complying with all relevant clinical governance standards, policies, and protocols Assisting in the management of common chronic medical conditions Recording clear and contemporaneous consultation notes to agreed standards Contributing to health needs assessment, research, baseline data collection, and clinical audit Compiling and issuing computer-generated acute and repeat prescriptions in accordance with Practice and Hampshire & Isle of Wight formulary guidance Maintaining professional learning and CPD requirements
