Associate GP

Shore Medical is seeking an Associate GP to join our primary care team, providing high-quality medical care to our patients. The post holder will provide a full range of GP services, including face-to-face and telephone consultations, home visits, care navigation, clinical assessment, diagnosis, treatment, prescribing and appropriate referral. They will manage a varied caseload, including patients with undifferentiated and complex health needs, while supporting health promotion, screening and disease management. We are looking for a GP who is committed to delivering safe, evidence-based and patient-centred care, working collaboratively with the wider practice team and contributing to clinical governance, audit, quality improvement and the ongoing development of primary care services.

Provide comprehensive GP services, including face-to-face and telephone consultations, home visits and care navigation.

Assess, diagnose and manage patients with a wide range of acute, chronic and undifferentiated health needs.

Prescribe safely, develop care plans and refer patients to appropriate services when required.

Provide senior clinical leadership, taking ownership of urgent operational issues and deputising for GP Partners when required.

Support, mentor and advise clinical and administrative staff, contribute to audit and quality improvement, and support the development of new models of care.
