Responsibilities Conduct thorough patient assessments, including medical history reviews and physical examinations Diagnose and treat a wide range of acute and chronic conditions with precision and care Manage medication administration, ensuring safe prescribing practices aligned with current guidelines Collaborate with multidisciplinary teams to develop personalized care plans for patients of all ages Provide specialized care in areas such as Care Homes, Frailty, Learning disabilities, paediatrics as needed Maintain accurate and detailed medical records in compliance with regulatory standards Educate patients on health promotion, disease prevention, and management strategies Participate in community health programs and public health initiatives to improve population wellness

We serve the Swarcliffe, Seacroft and surrounding area with 9300 registered patients. We have 3 GP Partners, 2 salaried GP's, ACP, Nursing team and clerical team. We are part of the Seacroft PCN.
