We are looking for a motivated GP to undertake the usual range of duties including surgery face to face consultations, telephone triage, video consultations, on-call, home visits (minimal) and the associated administration including but not limited to reports, path lab results, documents and prescriptions in a timely fashion. We offer chronic disease management clinics and have an excellent nursing, pharmacy and healthcare team who lead on this, enhanced services including minor surgery and sexual health. We have a successful dispensary with experienced staff and led by our in-house Pharmacist providing support and advice to patients and clinicians. We are high QOF achievers, using EMIS Web, training practice, member of local GP federation, and welcome any specialist interests. Supported by a frailty team of occupational therapists, pharmacists, social prescribers, wellbeing coach, physiotherapists and safeguarding care co-ordinators.
