About the role
We are seeking a consultant in Frailty to be a robust part of our emergency department and Frailty Assessment Unit alongside our other consultants. This will lead away from age related services towards an acute frailty service. Alongside this is an agreement to further implement an Acute Physician led AMU model supported by specialty in reach rotas. As part of this it is planned to change HCOOP referral from an age criterion to a frailty one.
Main duties for this role include: o Deliver high-quality care to older adults with a wide range of frailty-related conditions, including multi-morbidity, cognitive impairment, falls, delirium, functional decline, and complex geriatric syndromes.o Participate in multidisciplinary team (MDT) meetings focused on care planning, discharge coordination, and management of complex cases.o Contribute to the acute frailty service, inpatient geriatric wards, and outpatient clinics.o Be encouraged to take part in the Acute General Medicine on-call rota if able and willing.
