Patient Consultations Conduct face-to-face, telephone, and online consultations with patients. Assess, diagnose, treat, and manage acute and chronic health conditions. Prescribing & Medicines Management Prescribe safely and appropriately in line with current evidence-based guidance and practice protocols. Review repeat prescriptions and conduct medication reviews. Referrals & Coordination of Care Refer patients to secondary care and other services as clinically appropriate. Work collaboratively with other health professionals Offer advice and support on disease prevention, screening programmes, lifestyle modification, and immunisations. Record-Keeping & Administration Maintain accurate, up-to-date, and legible patient records using the practices clinical system. Complete reports, letters, and forms in a timely manner. Home Visits Undertake home visits when clinically required and agreed as part of the duty rota. Duty Doctor / On-Call Sessions Participate in the duty doctor rota to provide urgent, same-day care for patients. Teamwork & Practice Development Attend and contribute to clinical meetings, significant event analyses, and quality improvement activities. Support the practice in meeting QOF, Enhanced Services, and other contractual requirements. Training, CPD & Governance Keep up to date with clinical developments and maintain GMC revalidation requirements. Participate in clinical audit, peer review, and appraisal processes.
