About the role

Job SummaryHealthier West Wirral PCN is seeking a dedicated, experienced GP to join our team, working 5 sessions per week split between: Acute Visiting Service (AVS)- providing home visits.

PCN GP Hub- delivering same-day appointments for patients from all five member practices, booked into cross-organisational slots

This is an exciting opportunity to work in a centralised hub model while supporting innovative primary care delivery and improving patient access across the network.

Main Duties

Deliver home visits and acute care via the AVS to housebound and care home patients

Provide same-day appointments from the PCN GP Hub for patients across all five practices

Assess, diagnose, and manage a wide range of acute and urgent conditions

Work closely with the multidisciplinary PCN team, including Nurses, Physician Associates, ACPs, Pharmacists, and Paramedics

Contribute to improving access to care, reducing waiting times, and supporting clinical capacity across the network

Maintain accurate patient records and adhere to PCN clinical governance standards

Participate in team meetings, learning, and service improvement initiatives

Apply clinical judgment to determine the scope of patients seen within the AVS and Hub framework

Healthier West Wirral PCN supports a population of over 70,000 patients across 5 different Practices including Marine Lake Medical Practice, Myrtle Group Practice, Upton Group Practice, West Wirral Group Practice and Greasby Group Practice. We are an ambitious and forward thinking organisation that has already had much success building a large PCN team including Physician Associates, ACPs, Phamacists, Pharmacy Tech, Nurses, Care Coordinators and Paramedics.

Essential

Eligibility Criteria Qualified GP
Registration and Certification:
Be included in NHS Englands list of medical practitioners as per section 91 of the NHS Act 2006.
Hold full registration with the General Medical Council (GMC) on the General Practitioner Register, verifying eligibility to practise independently.
Professional Standing:
Not be prohibited from providing services due to regulatory breaches (e.g., NHS Performers List regulations).
Not be suspended from practice by the NHS or removed from the GMC list of medical practitioners.
Not be subject to any interim suspension orders from the GMC under section 41A of the Medical Act 1983.
Employment Terms:
Be employed on terms at least as favourable as those outlined in the Model Terms and Conditions of Service for a salaried general practitioner employed by a GMS practice.
Previous Employment:
No restrictions based on previous substantive GP employment.
Temporary Employment:
May be employed in temporary capacities (e.g., locum roles), but this must be explicitly agreed upon with the commissioner if applicable.
Other Requirements:
Have the legal right to work in the UK.
Hold appropriate medical indemnity insurance covering all professional duties.
Complete an enhanced Disclosure and Barring Service (DBS) check.
Possess a valid digital badge for secure system access and patient care delivery.
Be willing to engage in ongoing professional development in line with PCN and ARRS requirements.
