About the role
We are seeking an enthusiastic and motivated Clinical Pharmacist (Independent Prescriber) to join our large, dynamic, and forward-thinking GP practice. You will play a key role in delivering safe, effective, and evidence-based prescribing, working closely with GPs and the wider multidisciplinary team to improve patient outcomes.
This is an excellent opportunity for a pharmacist who is passionate about patient-centred care and is looking to develop their clinical role within primary care.
Undertake clinical medication reviews for patients with complex needs, including those with long-term conditions and polypharmacy Optimise prescribing to improve patient outcomes and reduce risk Manage repeat prescribing systems and undertake medicines reconciliation, including post-discharge reviews Monitor patient responses to medication and adjust treatment plans accordingly Support the implementation of NICE guidance and local prescribing policies Contribute to QOF, clinical audits, and medicines safety initiatives Provide prescribing advice and support to GPs, nurses, and the wider practice team Respond to MHRA alerts and ensure implementation of safety guidance Ensure compliance with CQC standards and clinical governance requirements
