About the role

A new Care Home has opened in our PCN area and to continue to provide great care for patients, we are recruiting a GP to help by providing a weekly Ward Round at the new Care Home.

By providing a proactive and holistic service, the GP will help improve the health and well being of the patients whilst being supported by the PCN, practice staff and other specialist services. As we move towards Neighbourhood working, part of the role will involve linking with and developing the service to align with the goals of the Fit for the Future: 10 Year Health Plan for England. The ward round will generally take place face-to-face in the morning, with dedicated time in the afternoon to complete any referrals, liaise with practices or other care providers and undertake additional tasks related to the wellbeing of the Care Home patients. This second session could be at a local practice or potentially remotely. There is some flexibility on the day of the ward round and could be Tuesday, Wednesday or Thursday. The role is currently funded for one year, but we hope to extend.

About the PCN: Newcastle Inner West PCN comprises 5 GP practices with a total population of approximately 54,500 patients. We have areas of above average deprivation and a diverse population. We are committed to improving the care and access for our patients and strive to reduce health inequality across the system. About the Care Home: The Care Home is made up of a mix of studio and 1 bed apartments. The majority of residents are aged 18-64 but they are licensed for ages 65+ as well. It is a mixed gender unit. They offer Outcomes Based Commissioning Packages so the idea is that residents will move out and into their own accommodation after a period of support and rehab. Residents will have mental health and/or neuro-disability and are likely to have challenging behaviours. The Care Home is not registered to provide Learning Disability care. It is not a drug and alcohol rehab centre but given the category of patients, some individuals may present with substance misuse issues. The Care Home has direct access to OT and Physio and Positive Behaviour Support (PBS) practitioners. There is a registered nurse on site 24/7 and they have staff ratios of 1:3 daytime and 1:5 nighttime.

Person specification

Essential

A good knowledge and interest in general practice and primary and community care.
Experience caring for frail or complex patients.
Some understanding of the emerging integrated care system arrangements and the implications for general practice.
Interest in frailty, elderly care, or integrated care models.
Strong interpersonal, communication, written and presentation skills.
Ability to quickly establish personal and professional credibility with colleagues and other key stakeholders.
Excellent organisational and time management skills.
Committed to own continuing personal development and an ability to support others to develop and progress.
Good communicator and motivator.
A collegial team spirit and desire to work with a range of stakeholders.
GMC-registered General Practitioner with license to practice.
Completion of GP specialty training (CCT).

Desirable

Knowledge of local health issues in own PCN.
Experience of working with a wide range of stakeholders across the health and social care system.
Experience in care home or community ward settings.
Leadership or service development experience within a PCN.
