Locum clinical pharmacist

We are currently seeking an enthusiastic and patient-focused Locum Prescribing clinical Pharmacist to join our well established team at Fordbridge Medical Centre in Surrey. This is a full time temporary position based in surgery Monday to Friday 37.5 hours per week. Preferable is experience working in a GP setting. Knowledge of Emis Web and Docman 10, experience of long-term condition management clinics.

The post holder will provide expertise in clinical pharmacy and medicines optimisation as part of a multi-disciplinary team providing care for patients from general practice, community sites including care homes and patients homes where required. You will be responsible for delivering care directly to patients, including those with single condition problems, long term conditions, respond to medicine related queries and medicine management on discharge/clinical letters. You should be trained in Controlled drug management. Ability to run the Prescribing Quality Scheme in the practice
